Adalimumab (Humira) is a recombinant human IgG1 monoclonal antibody specific for human tumor necrosis factor (TNF). Humira consists of heavy and light chain variable regions and human IgG1 k constant regions. Adalimumab is produced by recombinant DNA technology in chinese hamster ovary cells expression system. It is formulated as injection for subcutaneous route of administration. Humira is indicated for reducing signs and symptoms, inducing major clinical response, inhibiting the progression of structural damage, and improving physical function in adult patients with moderately to severely active rheumatoid arthritis, for reducing signs and symptoms of moderately to severely active polyarticular juvenile idiopathic arthritis in patients 2 years of age and older. It can be used alone or in combination with methotrexate, for reducing signs and symptoms, inhibiting the progression of structural damage, and improving physical function in adult patients with active psoriatic arthritis. It can be used alone or in combination with non-biologic DMARDs, for reducing signs and symptoms in adult patients with active ankylosing spondylitis, for reducing signs and symptoms and inducing and maintaining clinical remission in adult patients with moderately to severely active Crohn’s disease who have had an inadequate response to conventional therapy, indicated for reducing signs and symptoms and inducing clinical remission in these patients if they have also lost response to or are intolerant to infliximab, for reducing signs and symptoms and inducing and maintaining clinical remission in pediatric patients 6 years of age and older with moderately to severely active Crohn’s disease who have had an inadequate response to corticosteroids or immunomodulators such as azathioprine, 6-mercaptopurine, or methotrexate, for inducing and sustaining clinical remission in adult patients with moderately to severely active ulcerative colitis who have had an inadequate response to immunosuppressants such as corticosteroids, azathioprine or 6-mercaptopurine (6-MP), for the treatment of adult patients with moderate to severe chronic plaque psoriasis who are candidates for systemic therapy or phototherapy, and when other systemic therapies are medically less appropriate. Humira is also indicated for the treatment of active moderate to severe hidradenitis suppurativa (acne inversa) in adults with an inadequate response to conventional systemic HS treatment. It is also indicated in moderately to severely active polyarticular juvenile idiopathic arthritis (JIA) to reduce signs and symptoms in patients between 2 and 4 years. Humira is also indicated for the treatment of non-infectious intermediate, posterior and panuveitis in adults and pediatric patients 2 years of age and older. Humira is indicated for the treatment of non-infectious intermediate, posterior and panuveitis in adult patients who have had an inadequate response to corticosteroids, in patients in need of corticosteroid-sparing, or in whom corticosteroid treatment is inappropriate. Humira is indicated for the treatment of moderately to severely active ulcerative colitis in paediatric patients (from 6 years of age) who have had an inadequate response to conventional therapy including corticosteroids and/or 6-mercaptopurine (6-MP) or azathioprine (AZA), or who are intolerant to or have medical contraindications for such therapies. Humira is under development for the treatment of hidradenitis suppurativa, pyoderma gangrenosum and pustular psoriasis in Japan. It was also under development for peripheral spondyloarthritis in the EU and U.S, ankylosing spondylitis in China, axial spondyloarthritis and Behcet disease in the U.S, interstitial cystitis (painful bladder syndrome/bladder pain syndrome), Behcet disease and progressive sarcoidosis, Crohn’s disease in China.
