Overall, non-industry-sponsored trials outnumbered industry-sponsored trials. Moreover, compared to 2022, where non-industry trials steadily decreased for Q3 and Q4, the number of non-industry and industry-sponsored trials remained nearly level for all quarters. APAC conducted the highest number of clinical trials in 2023, with China recording the highest number of trials. This could be partially due to the high number of COVID-19 trials still ongoing in the APAC region.
The top primary intervention by an industry sponsor for innovator drugs in 2023 was pembrolizumab. Pembrolizumab is a monoclonal antibody that binds to the programmed cell death protein 1 (PD-1) receptor and blocks its interaction with programmed death ligand 1 (PD-L1) and PD-L2. This, in turn, releases PD-1 pathway-mediated inhibition of the immune response, including the anti-tumor immune response.

Completed Trials in 2023
Not all completed trials listed their endpoint status. Unlike in 2022, non-industry sponsors had the most trials achieving endpoints. Most of the universities that sponsored clinical trials were primarily focusing on indications such as oncology and the central nervous system. Shahid Beheshti University of Medical Sciences had the largest number of completed trials in 2023, followed by Tehran University of Medical Sciences.

Most clinical trial terminations occurred in Phases I and II. The main reasons given for trial termination were business or strategic decisions and product discontinuation. Oncology and central nervous system had the highest number of terminated trials for 2023.
Drugs like Milademetan tosylate, STSG-0002, and lotiglipron tromethamine had the most trial terminations for 2023. Milademetan tosylate was in the pipeline for the treatment of breast cancer, lung cancer, soft tissue sarcoma, liposarcoma, and lymphoma. It is administered orally.
